Workshop  – Service Specific Assessment & Care Planning 

Facilitator:  Kate Pascale & Associates

Date: Wednesday 22nd August

Time: 1pm – 4:30pm

This interactive forum will support staff to build on their assessment and planning skills.  We will discuss a range of practical ways to:

  • Build on the information that has already been collected to guide your assessment and planning
  • Consider ways to streamline their approach, without compromising quality.
  • Reduce duplication and improve efficiency for clients, carers and staff
  • Focus on what’s important to the client, their priorities and how you can best support them
  • Introducing the concept of goal setting and care planning to clients and carers
  • Engage clients and carers to identify relevant and meaningful goals
  • Create care plans that are meaningful and useful for you and your clients

Assessment and Care Planning Workshop – Putting Planning into Practice

“Putting Planning into Practice” workshops, developed by Wellness & Reablement Consultants, provide an opportunity for staff to sharpen their knowledge and skill in turning conversations into care planning that supports a wellness, reablement and restorative approach.

 

Workshops for Social Support – Make it Meaningful

Developed by Wellness & Reablement Consultants, these workshops provide an opportunity for staff to sharpen their knowledge and skill in turning conversations into care planning that supports a wellness, reablement and restorative approach. 

 

Workshops for Social Support – Next Steps: To Thinking and Planning

2 – 3 hour workshops are available for staff to sharpen their knowledge and skill in turning conversations into care planning, or what’s important for a client into activities that make a difference

This workshop is based on “Next Steps to Thinking and Planning”, a step-by-step process for thinking and planning new ways of offering social support activities. Staff are encouraged to consider:

  • how they gather client information
  • how this information is used to develop a person-centred care plan
  • how the care plan is considered in activity and program development
  • what the client achieves by attending social support
  • reviewing activities/programs to ensure they meet the clients’ needs
